A Clinical Observation of 28 Cases of Biliary Ascariases

Abstract
During the period of 5 years from July, 1966 to July 1971, 28 cases of biliary ascariases were seen
at Tae-Gu Fatima Hospital, and recent literatures were reviewed.
Following results were obtained.
1) The percentage ratio of biliary ascariaseis to the total number of biliary surgical patient during
this period was 16%.
2) The ratio of male to female of the patient was 1:1.3 and the predominant age incidence was in
the 4th decade of life.
3) 21 cases had previous attack of colicky pain on R.U.Q. in past history, and 17 cases accompanied
with cholelithiasis which were detected on operating table and these occupied 64% of all the patient.
Considering these concomittant cholelithiases, the migration of ascaris into the biliary tract is thought
to be more easily enhanced by the already present biliary pathology.
4) Mummified ascares were detected in 3 cases, one of which accompanied choledo-holithiasis and
another had hepatolithiasis.
5) Seven cases of biliary ascariases detected with post-operative T-tube cholaniography had been
absolutely negative in foregoing surgical exploration.
6) The worms deteced post operatively were successfully removed in 5 cases conservatively with
continuous suction through the T-tube or reinserted nelaton catheter and administration of 10%
piperazine solution through the T-tube.
7) Such surgical procedures as mechanical dilation of the sphincter, sphincteroplasty, and choledo
choenterostomy were thought to be good factors which enhanced biliary migration of ascaris, therefore
preoprative for parasites especially of round worms before performing the above mentioned procedures
are thought to have a good significance especially in highly contaminated area such as our country.
